Personal Transition Plan (PTP)

Who must complete the PTP?
All students on the diploma route must complete the PTP requirements to receive the .05 credit required for graduation.
 
What is the PTP, and what are the requirements?
The PTP is a .05 credit graduation requirement for the State of Hawaiʻi.  It is an individually designed and custom-tailored plan of action that helps guide each high school student's successful move from high school to post-secondary or career venues.  Individual PTPs will help identify available resources for students and include student self-assessment and inquiries into career choices.  
 
The four PTP requirements at Baldwin are...
  1. Post-Secondary Plan
  2. Personal Statement
  3. Professional Resume & Cover Letter
  4. Career Portfolio
    1. At least one artifact MUST be linked under Projects, Written Works, and Letters of Recommendation in the Career Portfolio.
Students can find all PTP requirements in the "Student - BHS PTP Information" Google Classroom (class code:  zvh5umuf, class invite link).
 
When is the PTP due?
Freshmen are required to complete the first draft of all PTP requirements, which will be revised, edited, and updated throughout their high school career.  The final drafts will be completed in junior year but will continue to be revised, edited, and updated through senior year.
 
The .05 credit needed for graduation will be issued to the student at the end of their Junior year if all requirements are completed.
 
Deadlines for SY 25-26:
  • Frosh, Sophs, and Juniors 
    • Juniors who do not complete their PTP by Friday, May 1, will not be eligible for Early Release or TA Options.
  • Seniors
    • Seniors must complete all PTP requirements by Friday, March 13, to participate in the graduation ceremony.
    • Seniors must complete all PTP requirements by Thursday, May 28, to receive the .5 credit required to obtain a diploma.
 
Where will students complete & submit their PTP requirements?
At Baldwin, students complete their PTP in various classes.
 
All assignments must be submitted in the PTP Google Form, which can be found in the "Student - BHS PTP Information" Google Classroom (class code:  zvh5umuf, class invite link). 
 
The homeroom teacher reviews Google Form responses where students submit assignments, mark them complete, and submit the completion checklist to the PTP Coordinator.
