Coordinator: 504 / Tier 2 Counselor / Gr 9-12

As the 504 Coordinator and Tier II Counselor, I perform the following duties: 

• Advocate for students identified under the Section 504 Disabilities Act. 
• Work with potential students and family in the evaluation, identification, and eligibility process under Section 504 regulations. 
• Develop student specific 504 Plans that meet student needs based on their identified disability. 
• Track and monitor student progress throughout their enrollment.
• Coordinate meetings and communication between 504 students, parents/guardians, teachers, and administrators. 
• Track and Monitor student attendance.
• Contact parents/guardians to discuss school attendance policy.
• Contact Complex area Social Worker and/or other DOE approved advocates for additional support based on our school attendance policy.
• Coordinate attendance review meetings with parents/guardians, students, and administrators.
• File truancy requests and court filings based on administrator recommendation.
